"NEW INSURANCE REQUIREMENTS EFFECTIVE IMMEDIATELY"

Proof of General Liability Insurance in the amount of $1,000,000 which includes the City as an additional insured is required. The endorsement must be submitted on the City's General Liability Special Endorsement  The form must be submitted with an original signature, copies are not acceptable. All endorsement forms or Insurance Policies must be approved by:
